Years of clinical experience in one of the best orthopedics and rheumatology  hospitals in the country prepared us to provide home care to patients with  bone and joint conditions.

We recognize the value of a Pre End Stage Renal Disease and ESRD Supplemental Care Program to help patients maintain optimum health and improve their quality of life. We have a program in place and have assisted patients manage their condition.

 

  Registered Nurse will assess patient’s understanding of disease condition, complications and management , ability to function in the home, living arrangement, extent of family involvement and community support, make referrals and coordinate care among members of the home health care delivery system.

Instruct patient and caregiver on pre and post-operative procedures.

Ensure medication compliance by teaching patients and caregivers on the purpose, proper administration and possible side effects.

Evaluate outcomes and notify the physician of patient ‘s progress.

           Physical Therapist will evaluate the patient and devise a program according to the physician’s plan of treatment. Restorative services include, muscle function training, and strengthening, teaching transfer techniques in all areas, gait training, and stair climbing.

           Occupational Therapist will evaluate and establish a program using therapeutic activities and adaptive devices, designed to assist the patient attain functional independence with activities of daily living and safety at home.

Speech Therapy

          Therapist provides valuable service to the patient and family.  This service includes evaluation of speech, language, and swallowing dysfunction, reporting findings and making recommendations to the physician. Counsel and instruct the patient’s family and home care staff in appropriate methods for restoring communication skills and aid in the recuperative process

Medical Social Worker

     Social Workers provide short term assistance in coping with lifestyle changes brought about by illness, grief counseling, and a link to various community resources.

Home Health Aide

       Health Aides provide personal care; assist with activities of daily living, nutrition, and homemaking task.  Our Home Health Aides are supervised by Registered Nurses to ensure compliance with established plan of care.

They assist the Registered Nurse in monitoring vital signs; identify changes in patient condition, and abnormal signs and symptoms.

Dietitian Services & Supplements

Phlebotomy &  Laboratory Services

        specimen collection is done in the home and processed in one of our associate laboratories

Medical Equipment & Supplies

    facilitate delivery of equipments like hospital bed, wheelchair, walker, cane,  shower chair, commode, and incontinent supplies.
