  • Registered Nurses coordinates care within the entire health care delivery system, working with institutional providers and primary physicians; continuously assess medical conditions; ensure medication compliance by teaching patients and caregivers on the purpose, frequency, proper administration, and possible side effects of medications.

    Perform or instruct patients and caregivers on clinical procedures including but not limited to the following:

    • Blood Glucose monitoring and Insulin injection

    • Ostomy care and supplies

    • Enteral Feedings

    • Telehealth / Carematix System (get more info at: www.carematix.com)

  • Registered Physical Therapists evaluate patients and devise a program geared towards rehabilitation and restoration of physical function including home safety evaluation, muscle strengthening, teaching transfer techniques in all areas, gait training, and stair climbing.

  • Registered Occupational Therapist evaluate and establish a rehabilitation program using therapeutic activities and adaptive devices, designed to assist the patient safely perform activities of daily living. Work on upper extremity function and strength.

  • Speech Therapists evaluate speech, language, swallowing, and cognitive deficits. Instruct patients on therapeutic exercises, and advice patient's family and home care staff in effective communication skills.

  • Aides provide personal care: Assist with activities of daily living, nutrition, medication reminders, light housekeeping tasks, and checking Vital Signs. They are supervised by Registered Nurses to ensure compliance with established plan of care.

  • Social Workers serve as a link to various community resources and provide short term assistance in coping with lifestyle changes brought about by illness.

  • Our staff will help facilitate delivery of devices and supplies including Hospital Bed, Wheelchair, Walker, Colostomy Supplies, Wound Care Supplies, Incontinence Supplies, and Diabetic supplies.
