When we receive an order or referral for home health services, we will do our very best to assign qualified nurses and therapists specific to your medical needs and personal preferences.
However, the health care system has been experiencing a shortage of Registered Nurses. Like the rest of the industry, Care Choice Home Health may not be able to provide services or meet patient’s needs in a timely manner in some locations.
After a hospitalization or a brief stay in a rehab facility, services must start within 24 to 48 hours from your discharge. If we are unable to begin services within the Medicare required time frame or that has been ordered by your physician, we will let you and your physician know so you can decide on whether to wait for availability or look for another agency.
Home health visits can be one to two times a week for every ordered discipline based on medical necessity. Visits are intermittent, not daily and can be limited depending on the insurance plan or reimbursement. We will inform you of the established frequency of visits upon start of care, and update you on plans to continue beyond the 60-day episode, or discharge.
In cases where a patient is under our services and we are unable to continue due to staffing reasons, we will assist you in transitioning to another agency, in coordination with your physician.
