  • Medication compliance, diet control, and monitoring for signs of congestive heart failure exacerbation are essential to keeping patients out of the hospital.

    Your Care Choice Nurse will assist you with your medications, help you identify foods that could worsen fluid retention or elevate blood pressure which causes heart failure exacerbation.

    Your nurse will communicate to your physician any abnormal changes in your condition so it can be addressed in a timely manner.

  • Proper treatment of a surgical wound is critical in preventing infection. Your nurse will train you or a caregiver on how to change dressings using sterile techniques.

    Chronic Leg or Foot Ulcers or deep tissue Pressure Ulcers takes time to heal. Using the appropriate dressing supplies is essential to expediting the healing process. Your nurse and your physician will work together to determine the best approach.

    Your nurse will continually assess wound status and update your physician.
